  • Petition for Dependency Status Change
    Use this form to request a change in your dependency status (from dependent to independent), for financial aid filing purposes.  This form is only available at the Financial Aid Office.
  • Petition for Reinstatement of Financial Aid (first level)
    Use this form if your disqualification is due to a low GPA (below 2.0) and/or low passing rate (below 67% of units passed).  This form is used for the Fall 2018, Winter 2019, Spring 2019 and Summer 2019 terms at LAMC.  (2nd level petition form)
  • Petition for Extension of Financial Aid (first level)
    Use this form if your disqualification is due to having attempted 150% or more number of units required for your academic program or having an Associate Degree (or higher) from outside the LACCD.  This form is used for the Fall 2018, Winter 2019, Spring 2019 and Summer 2019 terms at LAMC.  This form is an online form, accessible through the student portal, under the FINANCES MENU button, labeled at SAP Petition Form.
     
  • SEP Adjustment Request
    Use this form if you are requesting a change to your list of eligible courses, as provided when your Petition for Extension was approved. The form should be used when you need to "swap" classes (i.e., request to add HUMAN 001 and remove HUMAN 002). If you need to add additional classes due to errors in your Student Educational Plan or a change in your academic program, you will need to complete a new Petition for Extension of Financial Aid.

    Getting your financial aid disbursement is easy through the myLACCDcard by BankMobile! Once you've completed the financial aid process, your refund selection kit will be emailed to your LACCD assigned email account. If you already have a myLACCDcard you will use that to access future financial aid disbursement.

    How are financial aid refunds delivered to students?

    Bank Mobile will issue a myLACCDcard to students who must choose one of the following disbursement options:

    · Electronic deposit to the student's current bank (ACH): a method that allows students to enter their banking institution’s checking or savings account information.

    · Electronic deposit to a Bank Mobile account: a checking account will be linked to the myLACCDcard.

     

    How do I chose my refund preference?

    Bank Mobile will send an email to all LACCD enrolled students who applied for financial aid with a matching Student ID to the LACCD Student email. We recommend that students check their student email periodically. The email will contain a personal code and a link to a website with instructions to select your disbursement options.

    What is a Bank Mobile Vibe account?

    The Bank Mobile Vibe account is a fully functioning FDIC checking account with no minimum balance and no monthly fees. Selecting the Bank Mobile Vibe account allows you to access your financial aid disbursements quickly and easily with great benefits, such as:
    • Free internet banking features.
    • The ability to make purchases anywhere a Debit MasterCard is accepted.
    • The ability to withdraw cash with no fees at any network ATMs. As of May 1, 2016, Higher One customers have access to Allpoint®, the largest surcharge-free ATM network. Visit allpointnetwork.com to find one of 55,000 ATMs or download the Allpoint phone app.
    • Free mobile alerts about your Bank Mobile Vibe account.

    Is the myLACCDcard a Credit Card?

    The myLACCDcard is NOT a credit card. However, you can use it for purchases at all participating merchants that accept Debit MasterCard, get cash at ATMs, pay bills, and more.
  • How will I know when my financial aid disbursement has been deposited to myLACCDcard?

    When your disbursement has been deposited into your account, Bank Mobile will notify you via the email address you entered during activation. You can also view the details of your Vibe Account by accessing your statement online. Additionally, you can sign up to receive text messages sent directly to your cell phone with Mobile Alerts. Look for these options as you activate your myLACCDcard.
  • What if I don't want to open a Bank Mobile account? Can I just get it deposited to my own bank account?

    Yes, you may have your financial aid disbursement deposited to the bank of your choice. In order to have your refund deposited to your bank, follow the instructions on your email from Bank Mobile to select your refund preference. If you already selected a bank mobile account, you may change your preference by accessing your account, selecting "Refund Preferences", and then selecting ACH or refund to your own bank account.
  • What is the maximum amount of money that can be withdrawn from an ATM in one day?

    You can withdraw up to $500 per day.
  • What is the most I can spend in a day if I use myLACCDcard?

     The daily spending limit is $2,500. However, if you wish to spend a larger amount, you can contact BankMobile Customer Service to request an increase (1-877-524-3985, Monday through Friday 8:00am – 8:00pm EST).
  • How do I log in to view my Bank Mobile account?

    Visit www.bankmobilevibe.com. Log in using your email address and password. The password is what you created upon activating your myLACCDcard.
  • Do I need to have a myLACCDcard to choose my refund preference?

    No. You will only receive the myLACCDcard if you select Bank Mobile as your account for financial aid refunds.​
