	2009-2010 Board of Governors (BOG) Fee Waiver Application (ENGLISH) 2009-2010 Board of Governors (BOG) Fee Waiver Application (SPANISH)
	Use this form to apply for the BOG fee waiver. This form is used for Summer 2009, Fall 2009, Winter 2010, and Spring 2010 terms at Los Angeles College.
	2009-2010 Student Authorization/Questionnaire/Ed Goal Form
	All students applying for financial aid must complete this form.
	2009-2010 Federal Student Verification Worksheet
	All students whose application has been selected for verification (an * showing by the EFC number located on the upper right hand corner of the Student Aid Report), must complete this form.
	2009-2010 Academic Competitiveness Grant (ACG) Verification Form
	Students whose Student Aid Report indicate that they may be eligible for the Academic Competitiveness Grant will need to downloan this form. The form will need to be completed by both the student and their high school/cognizant authority to verify eligibility.
	2009-2010 Petition for Reinstatement of Financial Aid
	Use this form to petition for reinstatement of financial aid eligibility if you have been disqualified due to having a cumulative GPA below 2.0 and/or having completed less than 67% of your attempted units.
	2009-2010 Petition for Extension of Financial Aid
	Use this form to petition for continued financial aid eligibility if you have been disqualified due to having attempted 90 or more units, having an Associates degree, and/or having a Bachelor degree or higher.
